  • The Palliative Care Nurse Practitioner is responsible for providing primary care for a caseload of patients throughout their disease trajectory, from diagnosis through end of life care. The Palliative Care NP will work collaboratively with the Medical Director of the Palliative Care Service and other interdisciplinary team members to develop the service and will play an instrumental role in delivering care to patients and their families with life limiting illness along the disease continuum. As an advanced practice nurse, the NP has received specialized training and education in the management of the acute care of adult Hematology Oncology patients. By virtue of such preparation, the NP is uniquely qualified to meet the responsibilities of the role which include, but are not limited to:



    • Responsibilities include providing comprehensive medical and psychosocial evaluations of patients and their families, skilled and meticulous symptom management, with extensive involvement and participation in discharge planning and continuity of care efforts. Perform bedside palliative care patient/family evaluation
    • Prescribe medications including controlled substances to the extent delegated and licensed.
    • Order treatments and durable medical equipment as indicated
    • Perform other therapeutic or corrective measures as indicated, including urgent care
    • Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed
    • Assist in all facets of care coordination for palliative care referrals
    • Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided
    • Comply with applicable laws and regulations with respect to Collaborative Agreements
    • Initiate reimbursement for services rendered.
    • Provide training and continuing education for staff
    • Assist in development of clinical practice guidelines/standards in support of quality palliative care
    • Adhere to the practice of confidentiality regarding patients, families, staff and the organization

    Requirements:

    • Current licensure in the commonwealth of Massachusetts as a Nurse Practitioner
    • Master’s Degree in Nursing
    • Current certification as a nurse practitioner
    • Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred
    • Certification as Advanced Practice Nurse in Hospice and Palliative Care, or eligible within first year of employment
    • Excellent interpersonal and communication skills
