  • The case manager provides care coordination for a patient's hospital stay. Working with the healthcare team, the case manager monitors the LOS, the appropriate utilization of care, promotes quality, provides clinical review for the payers, and plans/implements the discharge. This role is carried out in collaboration with the social workers, considering the care and psychosocial needs of both the patient and family. Integration of the best in evidence-based practice, professionalism and customer service are foundations for the case manager's work. The case manager is supported by the director of the Case Management and Social Worker, with daily coaching of complex cases and ongoing educational support and development. Routine reports inform the case managers of their outcomes and assists them in evaluating their practice and how to accomplish next steps. The Case Management Department: upholds the current standards of case management practice, and creates an environment that promotes the individual growth and development of its staff.

    The applicant will need to manage a daily caseload which is specialty-based, in accordance with hospital/ Department:al policy and procedures and regulatory requirements. Optimize their work to accomplish the goals and strategic initiative identified for each fiscal year. Possess the ability to transfer clinical knowledge/skill to the care coordination of their patients/families. Utilize critical thinking to case manager and implement a safe, appropriate discharge. Have the ability to work with the case manager technology and clinical leveling criteria. Be willing to learn and expand skill set as new protocols and projects are initiated.

    Minimum edudcation requirement is a Bachelor's degree in Nursing. Prior experience in case management or a related field is required. Current Massachusetts registered Nursing License.
